  • Licensed in the state of IN as social worker (LCSW) or counselor (LMHC)
  • CPR Certification
  • Annual Fire/Electrical Safety Training

Organizational, verbal, interpersonal, patient relations, analytical, grammar/spelling, reading/comprehension of written instructions, ability to follow verbal instructions, and computer skills.


Knowledge of community resources for older persons, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care. Familiarity with medical terminology and conditions as these impact treatment and discharge needs. Familiarity with medical team roles and with the functioning of other hospital departments. Knowledge of effective individual and group communication techniques.


  • Master's degree in Social Work from an accredited Graduate School of Social Work or a degree that

      supports an alternative acceptable licensure.

  • LMSW or LCSW preferred LMHC and LMFT also acceptable.
  • Two to three years of clinical experience with older persons, and hospital or nursing home experience


  • Must meet any applicable state certification standards.

  • Exhibit communication skills necessary to interpret age specific response to situations and treatments
  • Ability to provide equipment necessary for age specific treatments
  • Exhibits knowledge of growth and development
  • Ability to provide age specific and interpret age specific treatments
  • Ability to involve family/significant other in patient's plan of care
