Essential Job Duties:
1. Accurate assessment and documentation of patient condition.
2. Demonstrates appropriate interventions related to patient condition
3. Demonstrates respect for patient rights by maintaining patient privacy, dignity, safety, confidentiality and right to refuse or consent to treatment.
4. Participates in interdisciplinary team conferences to assure common goals and interventions are communicated to multidisciplinary team.
5. Administers, teaches and gives instruction and direction in the utilization of medications in the home setting to assure the patient is receiving maximum benefit.
6. Demonstrates precision in the performance of treatments related to home care or seeks educational opportunities to become proficient.
7. Accurately provides patient/caregiver education to assure independence in the staff members' absence.
8. Keeps Manager informed of any unusual concerns, situations, events or conditions relative to staff, patients, families or physicians.
9. Documents all aspects of patient care and assures all documentation meets regulatory requirements, within set guidelines.
10. Reports problems/concerns regarding patients to RN in charge.
11. Demonstrates positive customer service skills with patients, family, M.D., peers and other departments.
12. Demonstrates ability to communicate positively with and supervise non-licensed staff related to patient care tasks.
13. Demonstrates appropriate infection control measures and standard precautions with delivery of patient care.
14. Thorough, timely, computerized documentation.
15. Demonstrates knowledge and compliance with AHC policies and procedures, acknowledges hospital administrative policies and procedures.
16. Functions independently with minimal supervision.
17. Possesses ability to plan, organize, develop, implement and interpret programs, goals and procedures necessary in the provision of care while demonstrating organizational and prioritization skills.

Under the supervision of the AHC Administrator, this position as a licensed practical nurse, bears responsibility and accountability for the nursing care patients receive within the realms of LPN scope of practice, nurses code of ethics and the Indiana Nurse Practice Act. Inherent in this practice is the nursing process, the assessment of the patient status, the plan of nursing actions as directed by the R.N., the implementation of that plan, and the evaluation of the plan. The AHC LPN must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to provide age-specific needs and to provide the care as described in AHC policies and procedures. The Home Health Care LPN bears responsibility for being knowledgeable of all applicable laws and regulation pertaining to the practice of home health nursing and adhering to same.
